The Nursing Process: My Secret For Free

In the midst of my frantic test preparation, I completely overlooked an important factor: the nursing process forms the foundation for ALL practice exam questions for NCLEX PN. Get ready to think back to your nursing school education!

The four phases of the practical nursing process are: data collection, planning, implementation, and evaluation. This process is at the heart of what actions nurses should always take. And when I applied this process to my NCLEX PN exam practice, I found that my test scores shot through straight the roof!

Need a reminder of what these stages entail? Don’t worry, I did too – and I’m passing this straight on to you:

  • Data Collection: In NCLEX PN practice, this refers to building a specific database for a patient. Collecting information involves interviewing the patient, the patient’s family and retrieving data from health records. This information is then used as part of the diagnosis process.
  • Planning: Once the relevant data for a patient has been collected, a practical nurse will then have to identify and plan out goals of individualized care.

  • Implementation: This is at the heart of the nursing process – because it’s where you as a nurse take action! From performing basic therapeutic care to providing a safe environment, it’s our job to make sure that patients are as safe and comfortable as possible.
